Why would the ground wires be cut in my house wiring?

My old house has BX (metal) cable running through it. Later someone put in a 100A main box and ran 3 wire NM cable to junction boxes which then hook up to the BX. But, they didn’t hook the ground wires to the boxes, or anything. In fact they cut them. Is there any reason this would have been done?

When the electrical wiring in an older house is, well, extremely inefficient, can it cause a fire?

Why isn’t surge protection built into the house wiring?

If electricians already know there is a chance for power surges, why don’t they put in some standard type of surge protector right at the circuit breaker box, so all the electricity coming into the house is the same?? Wouldn’t this make more sense than having to buy multiple surge protectors for every outlet and/or device in the house?
